Infusion Clinic Case Study

Expert Medical Advice on Safely Responding to Infusion Reactions

The Critical Moment

At a busy infusion clinic, a 63-year-old patient was undergoing her ninth infliximab treatment for ulcerative colitis and inflammatory bowel condition.  Although she had tolerated previous infusions without issue, this session took an unexpected turn. Midway through, she developed symptoms consistent with a reaction to the medication. The Registered Nurse, following established standing orders, administered IV Benadryl. This alleviated the patient’s symptoms. However, before resuming the infusion the Nurse needed expert guidance and reassurance to ensure the patient’s safety, and called PRAXES for support.

The Initial Assessment

The Nurse, experienced and well-prepared, was connected with a PRAXES Emergency Doctor by phone in less than a minute. The Nurse provided a clear summary of the situation, including the patient’s history and vital signs.

  • Temperature: Afebrile
  • Pulse: 98 bpm
  • Blood Pressure: 129/88
  • Oxygen Saturation: 98% RA

This was the patient’s first reaction to infliximab after several successful treatments. The Nurse sought approval to restart the infusion and advice on monitoring for further reactions.

The Recommended Treatment Plan

The PRAXES Doctor responded promptly and calmly, working collaboratively with the Nurse onsite. After reviewing the patient’s history, symptoms, and vital signs, the Doctor:

  • Approved the resumption of the infliximab infusion.
  • Provided clear guidance on what symptoms to watch for during the remainder of the session.
  • Outlined steps to take if the patient experienced another reaction, ensuring the Nurse was prepared for any scenario.

Throughout the process, the patient’s well-being was prioritized, and the Nurse was supported in delivering safe and effective care.

Positive Outcome with Minimal Disruption

Thanks to immediate telemedicine support, the patient’s condition improved with the care provided by the Nurse and real-time guidance from the PRAXES Emergency Doctor. This allowed the infusion to be completed safely during the scheduled appointment, avoiding the need to reschedule for another day or sending the patient to the Emergency Department.  The clinic avoided unnecessary delays, ensuring continued operations and effective patient care.
